Visit your NocoDB dashboard.
Open desired
project.
Click "Add new table".
Give the table a name.
Click "Submit".
Click "add".
Create a "Rating" field.
Switch it's type to "Rating".
Click "Save".
Click "add".
Create a "Remarks" field.
Switch it's type to "LongText".
Click "Save".
Create a "Form" view.
Give the form a name.
Click "Submit" button.
Click "Share View".
Check "Use Survey Mode".
Click "copy to clipboard" button.
Open the form from the copied link and submit it.
Get back to your NocoDB table. Close the "Share View" modal.
Switch to Grid view.
Enjoy feedback stored in your database.
